Identify the difficulties related to the diagnosis of Polycystic Ovary Syndrome (PCOS), the problems of access to both diagnosis and treatment and future perspectives related to this comorbidity. It consists of a narrative review of scientific literature searching for articles published from 2019 to 2024, in the following databases: PubMed/MEDLINE, LILACS, BVS and SciELO. For greater contribution in the search for articles, the ChatGPT tool was used. The narrative literature review aims to synthesize and critically analyze the content of the article, providing a comprehensive view of the topic. Diagnosing POP in women proves to be a challenge due to its complexity in the condition, lack of specific biological markers and overlapping symptoms of other diseases. In addition to delays in diagnoses, negligence and lack of awareness among the population on the part of health professionals and factors such as socioeconomic and racial inequalities that limit access to health care. Furthermore, the future prospects for the diagnosis and treatment of PCOS are promising with the use of new technologies.
